State law requires evidence of completion of an alcohol or drug education course as part of compliance for the 2nd or subsequent DUI conviction. The Department will need verifiable documentation that includes the following:
- On Certificate or Letterhead
- Specifies drug/alcohol/substance abuse
- Contains a Completed Date
- Includes the Driver’s personal identifying Info
- Contains a Signature of the instructor
