If your vision is less than 20/40 (worse than) in either eye, you will be provided a Tennessee standard vision statement to have completed by a licensed optometrist/ophthalmologist professional.
If you had a vision check-up in your former state within the past year, you can send this form to that professional to complete.
If not, you will need to locate a professional and have the form completed. We cannot refer you to an optometrist/ophthalmologist professional.
If the professional states your vision can be improved to the minimum 20/40 required standard with new lenses- you will need to wait until those lenses are received, before returning to our center.
Examples:
Left eye is 20/40, but right eye is 20/70, then a professional vision screening is required
Left eye is 20/40, but right eye is 20/200, which is considered blind - this means there is no chance of improvement with or without corrective lenses and no professional screening is required
