We only take documents uploaded via: Driver Services Online e-Services and submit the following documents:
- Affidavit Self-Certification
- Medical Examiner Certification (Form MCSA 5876)
If any submitted information is missing, unreadable, or incorrect, the medical certification team representative will notify the CDL holder as soon as possible. Otherwise, the CDL holder's license account information will be updated within 10 days per federal and state law.
Please do not attach your full long form medical report. We cannot accept your personal medical information for an update to your medical certification.
