- If you do not wish to visit a DSC you may notify the Department of your change of address by writing to us and giving us your:
- name (as it appears on your license)
- driver license number
- date-of-birth
- your address as it appears on your current license
- the new address you would like entered in your record
- a good daytime phone number in the event we need to contact you.
