If you need to go into a Driver Service Center:
If your parent is unable to be with you, they will need to sign the Teenage Affidavit of Financial Responsibility and have it notarized. If you did not get a compulsory school attendance form before school was closed, they will also need to sign the school waiver form and have it notarized.
You will need to have your certified birth certificate, proof of your social security number and 2 proofs of your name and address or your parent’s name and address.
The department can email these forms to you. Your parent will print them, sign them and have them notarized. Then you bring the completed forms, along with the other required documents when you come in for your test.
To avoid an extended wait time, making an appointment is highly recommended.
